This publication includes data for each GP practice in England about the prevalence of 20 health conditions among their registered patients. These include diabetes, depression, asthma, high blood pressure, chronic obstructive pulmonary disease (COPD), dementia, and epilepsy. These estimates are based on an analysis of 2022/23 data from England’s GP practices published by NHS Digital

Health Profile for England 2021 (Public Health England)
The Health Profile for England (HPfE) was first produced by Public Health England (PHE) in 2017. It brings together PHE data and knowledge with information from other sources to give a broad picture of the health of people in England today. The report has been updated annually, with content and format altering slightly each year.
The 2021 edition of the HPfE provides a comprehensive snapshot of the nation’s health and updates many indicators presented in previous reports. It also contains a summary of the impact of the coronavirus (COVID-19) pandemic on many aspects of health.

Public Health Outcomes Framework (Office for Health Improvement and Disparities)
The Public Health Outcomes Framework (PHOF) sets out a vision for public health to improve and protect the nation’s health, and improve the health of the poorest fastest. The framework includes a large number of indicators at a national, regional and local level that monitor different aspects of public health. These include overarching indicators around life expectancy, as well as more specific indicators for health improvement, health protection, healthcare, premature mortality and wider determinants of health.

This Office for Health Improvement & Disparities (OHID) tool has been developed to present evidence of health inequalities in England. Measures of inequality are provided for key indicators being used by OHID to monitor progress on reducing inequalities within England. For some indicators, inequality measures are also provided within regions, local authorities and NHS clinical commissioning groups. More local level measures will be added to the dashboard over time

The Subnational indicators explorer is an interactive tool that makes it easier for people to access and visualise key indicators at a local authority level. This is updated on a quarterly basis. The indicators are grouped into three categories (“boosting productivity, pay, jobs and living standards”, “spreading opportunity and improving public services” and “restoring a sense of community, local pride and belonging”) in line with the ‘Levelling Up the United Kingdom’ report. Local authority information and trends can be compared to other areas.
